Porocarcinoma is a very rare type of skin cancer arising from the eccrine sweat glands and poorly understood. The exact pathogenesis unknown. Clinical features are non-specific. It may present as painless, slow growing mass or nodule, ulcer, plaque, swelling, wart, papule naevus that increases in size over years to decades ulcerate bleed upon trauma. They occur equally both sexes most commonly found lower extremity. Though metastasis rare, they considered aggressive with high mortality rate. Diagnosis requires standard histopathological studies. Immunohistochemistry useful difficult cases. Treated early, porocarcinoma curable by wide excision. author reports case an extremity elderly lady summary its clinical, radiological histological findings illustrated.
